What's the Goal in Treating Type 2 Diabetes?

I'm going to talk about the  new guidelines  from the American College of Physicians regarding the treatment of adults with  type 2 diabetes . They published three basic articles around this topic. First was the clinical guideline, which I'm going to speak most about. Second was a  systematic review and network meta-analysis , which is basically the background from which the guidelines came. The third was a  cost-effectiveness analysis , which also had something to do with the final guidelines as well but isn't my particular strength when it comes to reviewing.  In terms of their meta-analysis, they basically included randomized controlled trials (RCTs) of a year or more, with 500 or more adult patients, which came to 130 publications. NEW ORLEANS — Medications in development would preserve muscle mass and augment fat loss when used in combination with glucagon-like peptide 1 (GLP-1) receptor agonists taken for weight loss. As drugs such as semaglutide (Wegovy) and the dual agonist glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 tirzepatide (Zepbound) are producing  unprecedented degrees  of weight loss in increasing numbers of people, concern has arisen about the proportion of the lost weight, approximately 30%-50%, that is beneficial lean body mass vs fat mass. Semaglutide Significantly Improves Chronic Kidney Disease

STOCKHOLM —  Semaglutide , a  glucagon -like peptide-1 (GLP-1) receptor agonist, has shown significant benefits in preventing major kidney disease, cardiovascular events, as well as mortality in patients with  chronic kidney disease  (CKD) and  type 2 diabetes  in the landmark FLOW trial. "This is the first trial that's specifically asked the question of whether this class of drugs — GLP-1 receptor agonists — protects the kidney and prevents major kidney outcomes, and the results clearly showed that that's the case," lead author Vlado Perkovic, MBBS, PhD, dean of medicine and health and scientia professor at UNSW Sydney, Australia, told  Medscape Medical News . Daily Marijuana Use Is Now More Common Than Daily Alcohol Use

May 23, 2024 – More people in the U.S. now use marijuana nearly every day than the number of people who use alcohol daily or most days of the week.  In a new analysis published in the journal  Addiction,  researcher Jonathan Caulkins, PhD, a cannabis policy researcher and professor at Carnegie Mellon University ,  analyzed  survey data stretching back to 1979, which was the first time marijuana use was included in what is now called the National Survey on Drug Use and Health. Daily or near-daily reported use of marijuana and alcohol have both increased in the past 30 years, but the increase has been much sharper among marijuana users. In 2022, 17.7 million people reported using marijuana anywhere from 25 to 30 days per month, compared to just 900,000 daily or near daily users in 1992. Advisory Panel Votes Against Once-Weekly Insulin for Type 1 Diabetes

In a 7 to 4 vote, an advisory panel to the US Food and Drug Administration (FDA) determined that the potential benefits of the investigational once-weekly basal insulin analog icodec (Novo Nordisk) don't outweigh the increased risk for  hypoglycemia  in people with  type 1 diabetes .  The May 24 meeting of the Endocrinologic and Metabolic Drugs Advisory Committee was convened specifically to advise the FDA of the benefits versus risks of icodec in people with type 1 diabetes based on the results of ONWARDS-6, one of Novo Nordisk's  randomized clinical trials  of icodec.  Five other  ONWARDS trials  were conducted in people with  type 2 diabetes .
